First Visit

During your initial appointment, we will strive to get to know you as a patient, perform diagnostic x-rays, a hygiene exam, and one of our doctors will do his through exam. In some cases, treatment can be diagnosed that same day, though a complex medical history or treatment plan may require an evaluation and a second appointment to provide the best care.

To help us serve you, please provide the following information:

  • A list of your current medical prescriptions.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
  • Your referral slip and any prior X-rays (if applicable).

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken prior x-rays within one year, you may want to bring them with you.
